Abstract
An 85-year-old man presented to the emergency department with delirium. He was taking the anticonvulsant primidone to manage essential tremors. His estimated glomerular filtration rate was low at 52 mL/min/1.72 m2. A urine barbiturate drug screen was positive (cutoff concentration 300 μg/L). Gross examination of the urine revealed an unusual hazy glistening appearance with white masses (Fig. 1). Microscopic examination revealed hexagonal plate-like crystals (Fig. 1). ... ... The answers are on the next page. Crystalluria and glistening urine can occur with primidone overdose (1–5), the likely cause in this renally compromised patient. Primidone has low solubility, a contributing cause of crystal formation (1). GC-MS qualitative analysis confirmed primidone and the metabolite phenobarbital, which was detected in the barbiturate screen. Cessation of the drug resulted in disappearance of crystalluria and symptom resolution. Primidone and phenobarbital therapeutic drug monitoring is recommended to assist with dosing, especially in patients with impaired renal function.
