Intervenções terapêuticas no cuidado dos Transtornos Mentais Comuns na Atenção Primária à Saúde

Abstract
INTRODUCAO: Apesar da alta prevalencia (56%) dos Transtornos Mentais Comuns (TMC), especialmente ansiedade e depressao, na Estrategia de Saude da Familia (ESF)/Atencao Primaria a Saude (APS), sao pouco detectados, resultando em sobrecarga para pacientes, familias e comunidades, alem de prejuizos sociais e economicos. Disponibilizar cuidado de saude mental na APS pode reduzir essa sobrecarga. Isso exige metodologias de intervencao especificas, alternativas ao modelo biomedico tradicional e personalizadas para a APS, certamente incluindo acoes nao medicamentosas.OBJETIVOS: Identificar intervencoes terapeuticas para TMC adotadas na APS. Descrever objetivo, modo de funcionamento e resultados, analisando facilidades e dificuldades para se emprega-las na APS.METODO: Revisao bibliografica nao exaustiva realizada nas bases de dados e respectivas interfaces MEDLINE/PubMed e LILACS/BVS. Termos chave: APS, ESF, TMC, ansiedade, depressao, intervencao terapeutica/terapia, atencao psicossocial. Periodo: janeiro 2007 a dezembro 2011 (5 anos). Criterios exclusao: transtornos graves (psicose, esquizofrenia), alcool e drogas, setores de saude diferentes da APS. Criterios inclusao: artigos sobre experiencias reconhecidas e nao recuperados nas buscas. Criterios selecao: escolhidas diferentes intervencoes para garantir a abrangencia. Leitura para 1o recorte: titulos; 2o.: resumos e 3o: artigos completos.RESULTADOS PRELIMINARES: No Brasil, ha praticas envolvendo as seguintes metodologias: APOIO MATRICIAL, CLINICA AMPLIADA, PROJETO TERAPEUTICO SINGULAR, PROJETO DE SAUDE NO TERRITORIO e PACTUACAO DO APOIO. Encontram-se tambem intervencoes com GRUPOS TERAPEUTICOS. A TERAPIA COMUNITARIA destaca-se como modelo made-in Brasil empregado em escala nacional, visto que o Ministerio da Saude ja formou mais de 1.500 profissionais nos ultimos tres anos.No Canada, medicos de familia adotam a ABORDAGEM CENTRADA NA PESSOA. Outras intervencoes sao empregadas internacionalmente, tais como: TERAPIA INTERPESSOAL BREVE, TERAPIA DE SOLUCAO DE PROBLEMAS e TERAPIA DE REATRIBUICAO. Ha experiencias com INTERVENCOES REALIZADAS POR LEIGOS e INTERVENCOES POR TELEFONE.CONCLUSAO: Internacionalmente, ha uma variedade de intervencoes terapeuticas utilizadas para prestar cuidados de saude mental integrados na APS que apresentam bons resultados, mas permanecem desconhecidas no Brasil. Uma vez que nao ha saude sem saude mental, recomenda-se testa-las considerando-se as devidas adequacoes ao contexto local.
