Abstract
Denizli yöresinde anormal hemoglobinler ve beta talasemiler, T.C. Sağlık Bakanlığı Denizli Hemoglobinopati Merkezi verilerine göre % 3.5 oranındadır. Denizli ili, ülkemizde Hemoglobinopati Kontrol Programı uygulanan 33 il merkezinden bir tanesidir. Anormal hemoglobinler yöremizde toplum sağlığı açısından önemli kalıtsal hastalıklar arasında yer almaktadır. Denizli yöresinde gözlenen anormal hemoglobinler içerisinde Hb D-Los Angeles [β121(GH4)Glu→Gln], % 57.8 oranı ile en sık gözlenen anormal hemoglobindir. Diğer taraftan yöremizde Hb S [β6(A3)Glu→Val], Hb G-Coushatta [β22(B4)Glu→Ala], Hb E-Saskatoon [β22(B4)Glu→Lys], Hb C [β6(A3)Glu→Lys], Hb J-İran [β77(EF1)His→Asp], Hb Beograd [β121(GH4)Glu→Val] gibi hemoglobin türlerine de rastlanmaktadır. Hb D-Los Angeles [β121(GH4)Glu→Gln]’a ilişkin çalışma sayısı, bu anormal hemoglobin türünün Hb S [β6(A3)Glu→Val]’e oranla daha az görülmesi ve belirgin bir sağlık sorunu oluşturmaması nedeni ile az sayıdadır. Özellikle beta globin gen ailesi haplotip çalışmaları belirtilen nedenlerle yaygın değildir. Yayınlanmış tek çalışma PAÜ Tıp Fakültesi Biyofizik Ana Bilim Dalı tarafından yapılan ve dünyadaki dördüncü özgün odağı tanımlayan çalışmadır. Ülkemizde Hb D-Los Angeles [β121(GH4)Glu→Gln] olgularındaki beta globin gen ailesi haplotiplerine ilişkin, ayrıntılı veri bulunmamaktadır. Tez çalışmasında; yöremizde sıklıkla rastlanan Hb D-Los Angeles [β121(GH4)Glu→Gln] mutasyonu taşıyan bireylerde, ε-globin, G/A-globin, ψβ-globin, δ-globin ve β-globin genleri üzerinde bulunan toplam yedi odak için, beta globin gen ailesi haplotip analizi yapılmıştır. Elde edilen sonuçlara göre, yöremizde gözlenen olguların büyük oranda Akdeniz kuşağı haplotip I [+ - - - - + +] ile ilişkili olduğu saptanmıştır. Bu haplotipin normal popülasyonda da ilk sırada olması mutasyonun yerel haplotip üzerinde gelişmiş olduğunu düşündürmektedir.
