Abstract
A Picture Says a Thousand Words is new Paediatrics & Child Health column that focuses on images that pose a diagnostic dilemma or are dramatic illustrations of a condition. For more information about this column, visit www.cps.ca/en/pch/manuscripts A previously healthy 14-year-old boy presented with a 4-week history of an intensely itchy eruption on his left foot (Figure 1). There was no eosinophilia and blood total IgE level was normal. He lived in a village nearby a beach in Langkawi Island (an archipelago off the coast of Malaysia) where he often went playing bare-feet with his mates. The itchy rash was slightly raised and advanced slowly, forming a few snakelike tracks that prompted him to seek medical treatment. There were a few dogs at home, and he often took the dogs to the beach with him. Other family members were not affected. The diagnosis is cutaneous larva migrans which is a zoonotic infestation caused by penetration and migration in the epidermis of filariform larva of an animal hookworm through contact with feces of infected animals, mostly dogs and cats (1–3). The most common causative larva is Ancylostoma braziliense (hookworm of wild and domestic dogs and cats) followed by Ancylostoma caninum (dog hookworm). After the incubation period, the larva start migrating and wandering freely in the epidermis, resulting in the formation of a pruritic erythematous migrating serpiginous slightly raised track which is pathognomonic. The itching can become very intense and if scratched may allow a secondary bacterial infection to develop. Cutaneous larva migrans usually heals spontaneously over weeks to months (1). However, the severity of the symptoms usually causes those infested to seek medical treatment before spontaneous resolution occurs. Following treatment, migration of the larvae within the skin is halted and relief of the associated itching can occur in less than 48 hours (4,5).
