P1_185 The Steroid Tapering in ANCA Vasculitis Evaluation Study (STAVE) 2: a Systematic Review and Meta-Analysis

Abstract
Objectives: No randomized clinical trials (RCTs) have examined the timing of glucocorticoid (GC) discontinuation in ANCA-associated vasculitis (AAV, comprised of granulomatosis polyangiitis and microscopic polyangiitis) with regards to relapse prevention and considerable clinical equipoise exists. Previous meta-analyses demonstrated that early discontinuation may be associated with decreased relapse rates. This updated systematic review and meta-analysis includes 8 of the original studies with 16 additional studies, and extended long-term follow up. Methods: MEDLINE, EMBASE, Cochrane Clinical Trials, and the grey literature were searched. RCTs and cohort studies were included if glucocorticoid doses were specified a priori and patients were followed for a minimum of 18 months. Experts were contacted to identify missing studies. 24 studies met inclusion criteria and were analyzed by treatment group. Results: Follow up time ranged from 18 months to 12 years. The mean time to GC discontinuation across studies was 14.34 months with 13 studies discontinuing GC prior to one year after induction. Most studies had a target glucocorticoid dose of zero (15 studies). 11 studies discontinued glucocorticoids after at least 12 months.
