Abstract

El sindrome de Smith Lemli Opitz (SLO) es un error congenito del metabolismo del colesterol de herencia autosomica recesiva caracterizado por multiples malformaciones congenitas, dismorfia facial y retraso mental. Los objetivos de este trabajo fueron: desarrollar  tecnicas de laboratorio para el diagnostico bioquimico  y molecular del sindrome de SLO, caracterizarlo  clinica y geneticamente y establecer un protocolo para el diagnostico y la atencion de la enfermedad . Se realizaron ensayos con  cromatografia de capa fina (CCF) para probar su sensibilidad y  especificidad para el diagnostico. Se establecieron criterios de sospecha de la enfermedad, se capacitaron a los especialistas para elevar el diagnostico clinico y se realizo una pesquisa en instituciones de atencion a discapacitados mentales. Se establecio una colaboracion con la Universidad McMaster de Canada que permitio realizar los primeros analisis moleculares y posteriormente se estandarizo el estudio de cuatro mutaciones. Se probo la utilidad de la CCF en el diagnostico y se identificaron 21 pacientes. Entre las principales caracteristicas clinicas se encontraron el retraso mental o psicomotor, la sindactilia 2-3 de los pies, la dismorfia facial, la microcafalia, el retardo pondoestatural y las anomalias genitales en los varones. Se identificaron mutaciones en el 77% de los alelos, T93M y IVS8-1G>C representan el 70% de los alelos. Se realizo el diagnostico prenatal en tres parejas de riesgo. Los pacientes reciben tratamiento por una  nueva dieta en el Dietario Medico. Se desarrollo un algoritmo de trabajo con lo que se logro mejorar la atencion integral a enfermos y  familiares.
