158The Prevalence of Dementia and Cognitive Impairment in our Acute Medical Admissions

Abstract
Dementia is one of the most significant issues facing the over 65 population worldwide. Over 11% (490,000) of Ireland’s population is now over the age of 65. 10% of those over the age of 65 currently have a diagnosis of dementia. (1) This audit set out to identify the prevalence of dementia and cognitive impairment in this age group being admitted to our acute hospital. This audit was a retrospective analysis of a cohort (41) of patients over the age of 65 admitted medically to our institution on three randomly chosen dates over 2016 and 2017 (26/5/16 and 27/2/17 and 28/2/17). Inclusion criteria was based purely on age of 65 or older. All patients had a formal cognitive assessment by Occupational Therapy which consisted of a Montreal Cognitive Assessment (MoCA) and Activity of Daily Living (ADL) assessment. Of the 41 patients identified as being over 65 years 7 (9.7%) had previously diagnosed dementia. Of these, 6 were from nursing homes and 1 from home. 16 patients (39%) were newly identified as having some level of cognitive impairment based on MoCA score (scores <20) and OT functional assessment. 18 patients (43.9%) were identified as having no cognitive impairment. Of the 16 patients with a new diagnosis of cognitive impairment or dementia 5 (31.25%) were discharged to nursing home and 11 (68.75%) home with additional supports. In the setting of an acute medical illness the issue of cognitive impairment and dementia can exacerbate the recovery and discharge planning of patients. Although the numbers in our study are low, we have identified a very high prevalence, well above the national average in our patient group. Dementia identification and care in acute older patients is essential to allow for adequate staffing levels, access to specialist services and discharge planning. (2)
