Abstract

Pour 1000 naissances vivantes, 10 enfants naîtront avec un cœur ayant des problèmes de malformation, c'est-à-dire avec une cardiopathie congéni-tale. L'utilisation de la prostaglandine E1 (PGE1) chez le nouveau-né atteint de cette anomalie permet de le maintenir en vie, de mieux le stabiliser et de l'amener à une chirurgie cardiaque correctrice ou palliative. L'objectif de ce projet est de trouver une alternative au traitement intraveineux qui soit mieux adaptée au traitement des nouveau-nés pré-maturés durant le transport. Le choix d'une nouvelle voie d'administration s'est porté sur la voie transdermique compte tenu des différents avantages qu'elle peut apporter en terme de sécurité, de modulation et de contrôle de la libération et d'absorption du principe actif (PGE1). La comparaison de différentes formulations liposomales, liposomes conventionnels et prolipo-somes, sera effectuée, ainsi que de leurs méthodes de fabrications, proportions et compositions. Il y aura par la suite évaluation de leur stabilité, potentiel de lyophilisation et de stockage, taux d'encapsulation du principe actif et absorption percutanée au travers de différents modèles (membranes synthétiques, peau normale humaine et substituts cutanés à partir de cellules souches d'adultes et de nouveau-nés). Une étude des propriétés enzymatiques de la peau ainsi que l'interaction de cette dernière avec les transporteurs liposomaux sera parallèlement évaluée. La facilité d'administration permettra de mieux protéger les nouveau-nés, tout en apportant une nouvelle approche ayant des qualités pharmaceutiques importantes.
