303 The Utility of a Panoramic Digital Imaging System (Panoptiq) in the Detection of Immature Leukocytes in Peripheral Blood Smears

Peripheral blood smear (PBS) review by a pathologist may be clinically indicated in several circumstances, such as in the presence of immature leukocytes. However, conventional microscopic examination of PBS slides requires the reviewer to have access to the glass slide. Telepathology solutions allow for remote consultation, and here we explore the utility of panoramic digital images for blast cell identification in PBS. PBS glass slides, originally created for pathologist consultation due to the presence of blasts, were retrospectively selected at random for digitization. Representative areas of the slides, consisting of approximately 100 leukocytes per case, were scanned at 100x oil immersion by a pathology resident using a microscope with attached camera and the Panoptiq v4.0 scanning software (ViewsIQ, Richmond, BC, Canada). A hematopathologist reviewed the digital slides offsite using the Panoptiq web-based viewer. Following a two-week “wash-out” period, glass slides were evaluated by conventional microscopy. The presence of blasts and estimated blast count was noted for each modality, along with scanning and reviewing times. Sixty PBS slides were scanned at an average time of 8 minutes 5 seconds per case and totaled 10.56 gigabytes of data. Review time by the hematopathologist averaged 1 minute 29 seconds and 1 minute 45 seconds by digital and glass slide modalities, respectively. There was a majority agreement between the two modalities for identifying blasts, at 67%, with blast cells missed on 12 digital slides. Using a semiquantitative method, blast counts were underestimated in eight cases, while overestimates did not occur. Discrepancies for blast identification and estimation between the two modalities may be attributed to cases with low white blood cell counts, representative scanning, and the subjectivity of choosing these areas. Despite these limitations, the Panoptiq scanning system is a practical tool to send rapid PBS consultations.
