039 The Development of a Patient Reported Outcome Measure to Assess Sexual Impacts of Genital Psoriasis: GPSIS
Bibliographic record
Abstract
To develop a patient (pt)-reported outcome instrument to measure the impact of genital psoriasis (GenPs) on sexual activity and to establish its content validity. This cross-sectional, qualitative study involved pts with moderate-to-severe GenPs. A literature review was performed to document the impact of GenPs on sexual activity. Impacts were confirmed by interviews with 2 clinical experts. Concept elicitation interviews with GenPs pts were then performed followed by item generation, translation/cultural adaptation, and then cognitive debriefing with a second group of GenPs pts. 52 articles met the search criteria. Sexual functioning was discussed by 19 (36.5%) articles involving psoriasis or GenPs; of these, 3 (5.8%) and 1 (1.9%) articles discussed sexual functioning related to GenPs and related to both psoriasis and genital psoriasis, respectively. Among conference abstracts (N=44), the most common psoriasis-related concepts (each n=2, 4.5%) were dyspareunia, increased discomfort or worsening of symptoms after sex, and sexual dysfunction. Concepts were confirmed by clinical experts. Twenty pts with confirmed GenPs then participated in concept elicitation. Pts were located in the continental United States (US). During interviews, key concepts emerged, including worsening of GenPs symptoms after sexual activity, reduced sexual frequency due to GenPs, and avoidance of sexual relationships due to GenPs. Based on pt interviews, the 3-item Genital Psoriasis Sexual Impact Scale (GPSIS) was developed to assess the impact of GenPs on sexual activity avoidance: “were you sexually active”; “how often did you avoid sexual activity due to your GenPs”; and “rate the level (degree) of worsening of your GenPs symptoms during or following sexual activity.” Cognitive debriefing interviews were conducted following translation/cultural adaptation with a second group of pts with self-reported GenPs (n=50). Pts confirmed the utility, understandability, ease of use, and cultural appropriateness of the GPSIS in English (Australia, Canada; n=10) and in target languages (Belgium, Netherlands, Canada, Austria, Puerto Rico, USA, Turkey; n=40).

Teacher imitationNot calibrated prevalence, not ground truth. Human validation pending. The Gemma side is a direct model label for every work in the frame, read from the title-only record. The Codex side is a classifier learned from the 10,348 direct Codex labels and calibrated to design-weighted sample rates; fields without enough sample support carry no Codex call. Candidate is the union of the two sides; consensus is their intersection. These outputs are machine_predicted_unvalidated and are not human labels.

Machine scores (provisional)
The two teacher heads of the student model, read on this work. A score orders the frame for review; it never asserts a category, and the validation status ships verbatim with every row.
Baseline scores from an immature model (maturity gate not passed, 7 training rounds). Scores rank; they never assert a category.
