A Case of Malignant Peripheral Nerve Sheath Tumor of the Rectum Treated by Laparoscopic Surgery

Abstract
症例は82歳，男性．前立腺肥大症・高血圧・糖尿病で近医にて加療中に腎機能低下を指摘され，腹部CT検査にて直腸腫瘤を指摘されたため精査加療目的に当院内科へ紹介となった．下部消化管内視鏡検査，生検の結果肉腫と診断され，手術目的に外科へ紹介となった．リンパ節郭清を伴う腹腔鏡補助下低位前方切除術を施行した．術後経過は良好であり，術後18日目に退院となった．病理組織検査により悪性末梢神経鞘腫瘍（malignant peripheral nerve sheath tumor:MPNST）と診断された．術後1年7カ月が経過した現在，再発・転移は認めておらず引き続き外来にて経過観察中である．MPNSTは末梢神経から発生する，あるいは末梢神経への分化を示す悪性腫瘍で，その多くは体幹・四肢近位部・頸部に発生し，消化管に発生することは稀である．今回，稀な直腸原発MPNSTに腹腔鏡手術を施行した1例を経験したので，文献的考察を交えて報告する．
