Abstract

Hacettepe Universitesi Tip Fakultesi Ic Hastaliklari Anabilim Dali, Uzmanlik Tezi olarak hazirlanmistir. Ankara, 2017. Kirilganlik hospitalize yasli hastalarda yuksek prevelansla izlenen ve genellikle tanisi atlanilan bir geriatrik sendromdur. Bu calismada akut saglik sorunlariyla hospitalize edilen yasli hastalarda kirilganligin kolay uygulanabilir, pratik yontemlerle tespit edilmesi ve kotu saglik sonlanimlari uzerine etkisinin incelenmesi amaclanmistir. Calismaya Mart 2017 ile Eylul 2017 tarihleri arasinda Hacettepe Ic Hastaliklari Servisleri’ ne akut saglik kotulesmesiyle yatisi yapilan 65 yas ve uzeri toplam 57 hasta dahil edildi. Hastalarin kirilganlik olcumleri Edmonton kirilganlik skalasi (EKS) ve klinik kirilganlik skalasi (KKS) kullanilarak yapildi. Hastalara kapsamli geriatrik degerlendirme testleri uygulandi. Biyoelektrik-empedans analiz yontemi (BIA) ile kas kutlesi olcumu ve el sikma gucu ile kuvvet olcumu yapildi. Hastalar yatis suresi boyunca kotu saglik sonlanimlari acisindan takip edildi. Kapsamli geriatrik degerlendirme sonuclari her olcekle yapilan olcumlerde de kirilganlikla iliskili bulundu. Spearman korelasyon analizinde EKS ile diger testler arasinda anlamli korelasyon saptandi. Kirilgan hastalarda el sikma kuvveti olcumlerinin kirilgan olmayan hastalara gore daha dusuk oldugu ve EKS puanlariyla el sikma gucu testi degerleri arasinda anlamli korelasyon oldugu goruldu. Kirilgan bireylerde hastane yatis suresinin kirilgan olmayan bireylere gore daha uzun oldugu tespit edildi ancak bu fark istatistiksel olarak anlamli bulunmadi. Hastane takibi sirasinda 1 hasta mortalite ile sonuclanmis olup, bu hasta her iki olcum yonteminde de kirilgan hasta grubuna dahildi.
