Abstract

Neuropsychiatric symptoms (NPS) in older adults with mild cognitive impairment (MCI) increase the risk of developing Alzheimer's disease (AD) but the underlying neural mechanisms are unknown. As part of an ongoing dementia prevention trial, we examined the association between NPS relevant to depression and anxiety and AD-related structural neuroimaging biomarkers in older adults with MCI but no lifetime history of depression. We hypothesized that greater NPS severity would be associated with decreased MTL volume. Participants were 45 older adults (mean age 75(7.5) years, mean MoCA= 23.5(2.4)) with DSM5 Mild Neurocognitive Disorder without current or previous history of depression and who were enrolled in PACt-MD, a clinical trial combining brain stimulation and cognitive remediation for older adults at risk for AD. NPS were assessed using the informant-based Neuropsychiatric Inventory Questionnaire (NPI-Q) at baseline. High resolution T1-weighted structural MRI scans were acquired at 3Tesla. Regions-of-interest (ROIs) were extracted using FreeSurfer and included hippocampal (HC), amygdalar, and entorhinal cortex (ERC) volumes, and ERC-thickness. Correlational analyses were performed between ROIs and severity ratings for the following NPI-Q subscales: depression, anxiety, apathy, irritability, night-time behaviours, and appetite. Depression was the most frequently reported NPS (33%), followed by night-time behaviors (31%), irritability (24%), apathy (22%), appetite change (22%) and anxiety (13%). After applying a Bonferroni correction for multiple comparisons, only NPI-Q severity ratings for night-time behaviors remained significantly correlated with ROIs. These regions were bilateral HC, left amygdala and left ERC-thickness (all p ≤ 0.001). Preliminary findings indicate robust correlations between sleep disruption and MTL volumes in older adults with MCI but no lifetime history of depression. These findings extend the current literature which suggests a bidirectional relationship between sleep disturbance and AD pathogenesis, potentially through effects on accumulation of cerebral beta amyloid protein (1). Given there is evidence that melatonin may improve cognitive function and sleep quality in MCI (2), the present results support the need for further research on the potential impact of sleep disturbance and sleep interventions on progression of AD in older adults with MCI.
