Abstract
Amac: Calismanin amaci ataksi semptomlari olan bir olguda sanal gerceklik uygulamasinin etkisini arastirmaktir. Gerec ve yontem: Arac ici trafik kazasi sonrasi kafa travmasi teshisi ile takip edilen olgunun degerlendirmesinde spinoserebellar ataksiyi isaret eden denge kaybi, dizartri, tremor problemleri bulunmustur. 2 yil sureyle tedaviye alinan hastanin progresyonunda plato evreye ulasildigi icin sanal gerceklik uygulamasi hasta tedavisi icin planlanmistir. Degerlendirmelerde ataksi ile ilgili olarak dengesel olan ve olmayan koordinasyon testleri, fonksiyonel uzanma testi, performans analizi icin Kanada Aktivite Performans Olcumu (Canadian Occupational Performance Measure – COPM) kullanildi. Sanal gerceklik uygulamasi icinde yer alan skorlar kaydedildi. Olgunun rehabilitasyon programinda amaca uygun icerige sahip iki oyun secilerek haftada bir seans, 30 dakika boyunca sanal gerceklik tedavisi uygulandi. Sonuclar: COPM’de olgu tarafindan belirtilen gunluk yasam aktivitelerindeki 7 aylik cocugunu kucagina alip yurumek, alis verise gitmek, tras olmak istedigini belirtti ve performans puanlarini sirasiyla 5, 5 ve 4 olarak; memnuniyet puanlarini ise 1 olarak bildirdi. Uygulama sonunda statik cift bacak durus suresi 30 sn’den 2.15 dk’ya; fonksiyonel uzanma testi sonuclari sol el icin 18 cm’den 22 ve sag el icin 19’den 23’ye arttigi gorulmustur. Ancak tek bacak durus suresinde 2 saniyelik gerileme, COPM sonuclarinda da degisme olmadigi kaydedilmistir. Tartisma: Olgumuza uygulanan sanal gerceklik uygulamasinin ataksi ile ilgili semptomlarin azalmasinda etkili oldugu dusunulmektedir. Denge, koordinasyon gibi vestibular sistem becerilerini artirmak icin uygun sanal gerceklik uygulamalari ile klinik olarak bireylere gelisme saglanabilir. Ancak kullanilan oyunlarin gunluk hayatta kullanilan aktiviteleri icermesi ile bireylerin rehabilitasyon surecinde gunluk hayata adaptasyonunu kolaylastiracagi dusunulmektedir
