L’établissement d’un poids précis, nécessaire pour obtenir un rétablissement physique, affectif et cognitif, fait partie des difficultés qu’affrontent les professionnels de la santé en pédiatrie auprès des enfants et des adolescents atteints d’anorexie mentale (AM). Les cliniciens et les chercheurs éprouvent de la difficulté à normaliser le processus, la méthodologie et la terminologie de ce qu’on appelle « l’objectif thérapeutique de poids » (OTP). Le présent point de pratique résume les recommandations tirées de méthodes courantes pour établir les OTP chez les enfants et les adolescents atteints d’AM, qui reposent à la fois sur des publications fondées sur des données probantes et sur un consensus d’experts. On y propose une approche personnalisée pour établir l’OTP, de même que certaines spécificités dans des situations cliniques particulières. De multiples facteurs orientent le processus menant à l’établissement d’un OTP pour un enfant ou un adolescent atteint d’AM, mais il est essentiel de leur accorder une attention personnalisée, particulièrement à l’égard des percentiles de poids, de taille et d’indice de masse corporelle ainsi que des courbes de croissance pédiatriques recensés pendant la période prémorbide. On souligne également la nécessité d’un suivi continu et d’une réévaluation régulière au sein de cette population.
