Efectos diferenciales de reserva cognitiva en poliusuarios de sustancias

Abstract
Se analizan diferencias en la relacion entre reserva cognitiva (RC) y desenlances neurocognitivos: ?Para que individuos la RC predice diferentes desenlances? No existe exploracion tal en la literatura sobre RC. Esto contribuiria a la identificacion de individuos en riesgo de deterioro neurocognitivo inducido por sustancias en funcion de patrones de RC. Identificar clases latentes basadas en los efectos diferenciales de RC sobre variables neurocognitivas, y determinar el grado en que el uso de sustancias predice pertenencia a clase. Hipotetizamos diferencias entre subgrupos latentes basadas en efectos que componentes de RC tienen sobre variables neurocognitivas, y que el consumo de sustancias modificara la probabilidad de pertenencia a clase. 166 hombres de centros para tratamiento de adicciones en la Ciudad de Mexico. Instrumentos: Evaluacion Cognitiva Montreal para estatus neurocognitivo; movimientos y tiempo de iniciacion de la Torre de Hanoi para secuenciacion visuoespacial e inhibicion conductual; Cuestionario Indice de Reserva Cognitiva para involucramiento en actividades relacionadas con educacion, trabajo y ocio; y entrevista para uso de sustancias. Realizamos tres modelos independientes de mixturas de regresiones con RC como predictor y variables neurocognitivas como desenlaces, y estimamos los efectos sobre la membresia de clase de los covariados edad de inicio, anos de uso, y uso de sustancias en mes previo. Se hallaron dos clases como mejores soluciones para los tres modelos (p. ej. para el estatus neurocognitivo, una clase donde solo la educacion sirvio como predictor y otra clase donde todas las dimensiones de RC predijeron el estatus). Edad de inicio, anos de uso, y uso de inhalables, alcohol y marihuana en mes previo, aumentaron las probabilidades de pertenecer a ciertas clases latentes. Individuos en riesgo de deterioro neurocognitivo inducido por sustancias pueden identificarse no solo por patrones de uso de sustancias sino tambien por la asociacion entre estos patrones e indicadores de RC.
