Introducao: para otimizar a drenagem toracica e diminuir o tempo de permanencia do tubo no espaco pleural, estrategias como uso de pressao positiva nao invasiva tem sido usadas e diferentes niveis de pressao foram testadas, como 7, 8 e 10 cmH2O com aplicacao variando de 30 minutos ate 4 horas. A tomografia computadorizada pode ser usada para avaliar qual nivel de pressao positiva nao invasiva e capaz de expandir mais area pulmonar de pacientes com drenagem toracica por derrame pleural, podendo ajudar nas melhores escolhas terapeutica na pratica clinica. Objetivo: comparar a expansao pulmonar sob 0 (zero) cmH2O (respiracao espontânea), 4 cmH2O e 15 cmH2O de pressao positiva nao invasiva em pacientes com derrame pleural drenado. Metodos: experimental randomizado. Envolveu dez pacientes com mais de 18 anos que desenvolveram derrame pleural apos trauma toracico penetrante, drenados dentro de 24 horas com dor controlada (abaixo de 3 pontos na escala visual analogica) e sem contra indicacao para uso de pressao positiva nao invasiva. Foram excluidos pacientes em uso de drogas vasoativas, com arritmias complexas, obstrucao de via aerea superior ou trauma de face, tosse ineficaz, distensao abdominal, nauseas, vomito e hemorragia digestiva alta. Cada paciente foi submetido randomicamente a tres niveis de pressao: 0 (zero), 4 e 15 cmH2O para avaliacao aguda da expansao pulmonar por meio da tomografia de torax (Aquilion 64-Toshiba Medical Systems®, Japan) sem contraste. A area pulmonar analisada foi medida no corte central do parenquima pulmonar por meio do software adequado para medicao de aerea, o RadiAnt DICON. Estatistica: a area pulmonar foi comparada usando o teste Friedman seguido do teste Tukey como post-hoc usando o software SigmaPlot 12.5. Resultados: a area pulmonar submetida a pressao de 15 cmH2O foi maior do que 4 e 0 cmH2O (mediana 3913mm2; IQR (Intervalo Quartilico), 3416-4390mm2 vs mediana 3495mm2; IQR, 3075-3954mm2 vs mediana 3382mm2; IQR, 2962-3658mm2; pConclusao: a pressao positiva nao invasiva de 15 cmH2O promove maior aumento de expansao pulmonar em area do que 4 cmH2O em pacientes com derrame pleural drenado. A pressao positiva nao invasiva de 4 cmH2O nao expande mais area pulmonar do que respiracao espontânea (zero de pressao positiva).
