As praxias visuoconstrutivas podem ser uma das funcoes cognitivas comprometidas na demencia, com avaliacao por meio da copia de desenhos. O instrumento de rastreio Montreal Cognitive Assessment (MoCA) avalia essa habilidade por meio da copia de um cubo. Este estudo teve como objetivo analisar o subteste do Desenho do Cubo do MoCA e descrever a sensibilidade e especificidade em idosos com diagnostico de Comprometimento Cognitivo Leve e doenca de Alzheimer, alem de disponibilizar novos escores para o MoCA. Conduziu-se um estudo de corte transversal, com 119 pacientes idosos, acima de 60 anos e com pelo menos 5 anos de estudo. Foram avaliados pelo Cambridge Cognitive Examination (CAMCOG) e Montreal Cognitive Assessment (MoCA). A aplicacao do MoCA foi duplo-cega. A analise do Cubo foi feita de duas maneiras: (1) escore do MoCA original (correto “1 ponto”; incorreto “0 ponto”); (2) pela analise de erros de cada desenho (Rotacao, Perseveracao, Fragmentacao, Dificuldade de Angulacao ou Alinhamento e Incoordenacao motora). Ao adicionar o sistema de pontos por tipo de erro no escore total do MoCA (a qual denominamos de MoCA Visuoconstrutivo – MoCA-VC) este sistema de correcao por tipos de erros (MoCA-VC) evidenciou maior sensibilidade na diferenciacao dos grupos. O MoCA-VC apresentou diferencas estatisticamente significativas (p< 0.0001) entre idosos saudaveis daqueles com doenca de Alzheimer (AUC= 1.000) e Comprometimento Cognitivo Leve (AUC= 0.970). O subteste do Desenho do Cubo do MoCA merece ser revisado considerando a correcao dos cinco erros com o objetivo de aumentar a acuracia diagnostica evidenciada pela praxia visuoconstrutiva.
