Tremblement mésencéphalique : apport d’une stratégie d’injection de toxine botulique guidée par enregistrement polygraphique

Abstract
Introduction : nos objectifs etaient de caracteriser les tremblements proximaux des membres superieurs a partir d’un enregistrement polygraphique et de montrer qu’une strategie d’injection guidee par enregistrement polygraphique etait efficace. L’evaluation de l’effet reposait sur l’echelle Fahn Tolosa Marin modifiee (FTMm) et sur la Canadian Occupational Performance Measure (COPM). Patients et methodes : nous avons analyse les muscles impliques dans le tremblement mesencephalique a partir de l’enregistrement polygraphique pour guider la strategie d’injection de toxine botulique. Les patients etaient revus a un mois pour evaluer l’efficacite et la tolerance de ce traitement. Resultats : nous avons inclus onze patients dans l’etude (huit femmes et trois hommes). La majorite (82%) avait une sclerose en plaques. Le tremblement etait present depuis 8 ans en moyenne. Notre etude a permis de montrer l’implication importante du deltoide dans le tremblement du membre superieur et notre travail montre une diminution significative du tremblement a un mois d’injection sur l’echelle FTMm (50,6 ± 4,32 ; 43,2 ± 4,59 ; p= 0,042) et de la cotation clinique specifique au membre superieur (16 ± 6,0 ; 11 ± 5,7 ; p= 0,043) sans modification significative de la COPM. Un patient a developpe une faiblesse transitoire du bras droit. Conclusion : l’enregistrement polygraphique modifie la strategie d’injection de toxine botulique avec un benefice sur le tremblement du membre superieur.
