Les dissections aiguës de l'aorte constituent une urgence médicochirurgicale dont la mortalité et la morbidité restent élevées en dépit d'importants progrès dans le diagnostic et le traitement. Le diagnostic clinique peut être difficile, notamment dans les formes atypiques, et doit conduire à la réalisation d'examens complémentaires en urgence devant toute suspicion de dissection aiguë de l'aorte. Les biomarqueurs, en particulier la mesure des D-dimères, associés à des scores de risques de dissection aortiques permettent parfois d'évoquer le diagnostic mais semblent surtout intéressants pour leurs valeurs prédictives négatives. L'utilisation respective des procédures d'imagerie diagnostique dépend essentiellement de leur disponibilité en urgence. L'échographie transthoracique chez les patients instables et surtout l'angioscanner dès qu'il est réalisable prennent une place prépondérante dans le diagnostic. Le bilan de la maladie doit permettre d'établir une classification anatomique précise. La localisation de la porte d'entrée intimale, l'extension détaillée de la dissection avec l'individualisation de défauts de perfusion d'aval, ainsi que la recherche d'une insuffisance aortique et d'un épanchement péricardique sont tous des éléments primordiaux. Le traitement médical doit être institué précocement dans tous les cas et repose essentiellement sur le contrôle de la pression artérielle et sur l'analgésie. Le traitement chirurgical connaît des évolutions importantes justifiant un transfert rapide vers des centres spécialisés. Les dissections de l'aorte de type A relèvent d'un traitement chirurgical en urgence. En cas de dissection aortique de type B, en l'absence de complications, le traitement de première intention est le plus souvent médical. Toutefois, le développement des endoprothèses aortiques est à l'origine de stratégies de prise en charge nouvelles ayant pour but de traiter mais aussi de prévenir la survenue des complications secondaires.
