Abstract
Uvod:Perkutanni transluminalni koronarni angioplastika (PTCA) kmene leve koronarni tepny (LMCA) nechraněneho chirurgickou revaskularizaci (CABG), byla v minulosti považovana za kontraindikovanou. Přiznive primarni i dlouhodobe výsledky PTCA chraněneho kmene vedly k elektivni PTCA nechraněne LMCA u pacientů, kteři byli kontraindikovani k CABG. Řada praci referuje o vysoke primarni uspěsnosti. Riziko restenozy LMCA je kolem 20 %. PTCA nechraněneho LMCA je dnes považovana za relativně bezpecnou alternativu CABG u vybraných skupin pacientů. Cil:Retrospektivni analýza výkonů na LMCA provedených v katetrizacni laboratoři Kardiocentra VFN ve dvouletem obdobi 1998-1999. Výsledky:Ve sledovanem obdobi bylo provedeno celkem 1 078 PTCA. Výkony na LMCA byly provedeny u 17 (1,6 %) pacientů, z toho u 13 (76 %) se jednalo o elektivni výkon na chraněnem kmeni - skupina A - a ve 4 (24 %) připadech o přimou PTCA nechraněneho kmene u akutniho infarktu myokardu (AIM) - skupina B. Vsichni intervenovani byly muži. Průměrný věk ve skupině A byl 62,6 roků, leva tepna byla chraněna žilnim nebo tepenným bypassem, důvodem intervence byla angina pectoris 3.-4. stadia podle Canadian Cardiovascular Society (CCS). Primarni uspěsnosti bylo dosaženo u 12 (92 %) pacientů, stent byl implantovan 11 (85 %) pacientům. Dva pacienti byli pro restenozu do půl roku uspěsně intervenovani znovu. Ve skupině B se jednalo o 4 pacienty s AIM s uzavěrem kmene leve tepny. Průměrný věk byl 51 let, 2 pacienti byli v kardiogennim soku. Intervence byla primarně uspěsna ve 3 připadech, kdy byl implantovan stent; jeden pacient v sokovem stavu přes pocatecni uspěch zemřel během hospitalizace, dva nemocni podstoupili uspěsný CABG. Zavěr:Pocet PTCA LMCA je na nasem pracovisti relativně velmi malý. Dobre výsledky PTCA chraněne LMCA jsou předpokladem k elektivni PTCA nechraněne LMCA u skupin pacientů kontraindikovaných k CABG.
