MEDULLA SPİNALİS YARALANMASI GEÇİREN VE ŞİDDETLİ NÖROPATİK AĞRISI OLAN BİR VAKADA DİADİNAMİK AKIM UYGULAMASI

Abstract
Bu vaka sunumunun amaci, medulla spinalis yaralanmasi (MSY) geciren ve siddetli noropatik agrisi olan ancak medikal tedavi ve Transkutanoz Elektriksel Sinir Stimulasyonu (TENS) uygulamasindan fayda goremeyen bir vakada diadinamik akim uygulamasinin etkisini incelemekti. MSY sonrasi cerrahi tedavi geciren ve yogun bakim unitesinde kalan hasta yaralanmadan yaklasik iki ay sonra rehabilitasyon merkezimize basvurdu. Hikayesi alinan ve fonksiyonel fizyoterapi degerlendirmeleri yapilan hastaya fizyoterapi programi planlandi. Fizyoterapi programi devam ederken hastanin yatis sirasinda da var olan ve ust ekstremitelerinde lokalize agri, siddeti artarak onemli bir sikâyet haline geldi. Uzman hekim tarafindan noropatik agri tanisi konan hastaya medikal tedavi planlandi ve fizyoterapi programina TENS eklendi. Ancak aradan gecen uc ay sureye ragmen hastanin agrisinda herhangi bir azalma olmadigi gozlendi. Bunun uzerine ganglion blokaji sagladigi dusunulen ancak kanit duzeyi dusuk olan diadinamik akim uygulamasi yapildi. Tedavinin ilk gununden itibaren agrisinin azaldigini belirten hastanin, toplam 40 puan olan “McGill Agri Olcegi” sonucu 10 seans sonunda 24 puan olarak bulundu. Sonuc olarak; medikal tedavi ve TENS gibi sikca kullanilan agri giderici ajanlardan fayda gormeyen bu vakada 10 seanslik diadinamik akim uygulamasinin agriyi azalttigi belirlendi.
