Abstract
Es liegt wenig Evidenz zum Vergleich der kardiovaskulären (KV) Sicherheit und des Risikos für schwere Hypoglykämien von Basalinsulinen bei älteren Patienten mit Typ 2 Diabetes (T2D) vor. Diese sekundäre DEVOTE-Auswertung untersuchte die KV Sicherheit und das Risiko für schwere Hypoglykämien von Insulin degludec vs. Insulin glargin 100 E/ml bei älteren Patienten. Die Daten aus DEVOTE (einer randomisierten, doppelblinden Treat-to-Target-Studie, in der 7.637 Patienten mit T2D und hohem KV Risiko über einen Median von 2 Jahren beobachtet wurden,) wurden nach Alter stratifiziert (50–64 Jahre [n = 3682], 65–74 Jahre [n = 3136] und ≥75 Jahre [n = 819]). HbA1c, Nüchternplasmaglucose und geschätzte GFR waren mit zunehmendem Alter niedriger. Ältere Patienten hatten ein höheres Risiko für bedeutende unerwünschte kardiovaskuläre Ereignisse (MACE), Gesamtmortalität und schwere Hypoglykämien, unabhängig von der Behandlung. Mit Insulin degludec vs. Insulin glargin 100 E/ml war das Risiko für das Eintreten eines MACE (Hazard Ratio [HR] 50–64 Jahre: 0,84 [0,67; 1,05] 95% KI; 65–74 Jahre: 0,90 [0,72; 1,14] 95% KI; ≥75 Jahre: 1,24 [0,82;1,88] 95% CI) wie auch für die Gesamtmortalität vergleichbar (HR 50–64 Jahre: 0,88 [0,64; 1,22] 95% KI; 65–74 Jahre: 1,04 [0,78; 1,38] 95% KI; ≥75 Jahre: 0,78 [0,52; 1,19] 95% KI). Jedoch war die Behandlung mit Insulin degludec mit einem geringeren Risiko für schwere Hypoglykämien gegenüber Insulin glargin 100 E/ml assoziiert (Risikoverhältnis 50–64 Jahre: 0,59 [0,42; 0,82] 95% KI; 65–74 Jahre: 0,59 [0,41; 0,84] 95% KI; ≥75 Jahre: 0,74 [0,37; 1,46] 95% KI). Die Auswertung unterstützt die KV Sicherheit von Insulin degludec, mit geringerem Risiko für schwere Hypoglykämien gegenüber Insulin glargin 100 E/ml bei älteren Patienten mit T2D und hohem KV Risiko.
