Wirksamkeit und Verträglichkeit von Dapagliflozin bei Patienten mit Typ-2-Diabetes und moderater Niereninsuffizienz (Chronische Nierenerkrankung Stadium 3A): Die DERIVE-Studie

Abstract
Fragestellung: Der selektive SGLT-2 Inhibitor Dapagliflozin (DAPA) verursacht Glukosurie und senkt die Blutglukose bei Typ-2-Diabetes-(T2D)-Patienten. Das Ausmaß der Glukosereduktion hängt von der Nierenfunktion und der filtrierten Glukosemenge ab. Eine Verminderung der glykämischen Wirksamkeit von SGLT-2 Inhibitoren bei T2D-Patienten mit moderater Niereninsuffizienz (chronische Nierenerkrankung [CKD]-Stadium 3; eGFR 30–59 mL/min/1,73m2) wurde zuvor gezeigt. Diese Patienten zeigten positive Auswirkungen auf Körpergewicht und Blutdruck. Methodik: DERIVE (NCT02413398) randomisierte 321 Patienten mit T2D (HbA1c 7–11%) und moderater Niereninsuffizienz (CKD-Stadium 3A; eGFR 45–59 mL/min/1,73m2) und untersuchte Wirksamkeit und Verträglichkeit von 10 mg DAPA gegenüber Placebo (PBO) über 24 Wochen. Primärer Endpunkt war die mittlere Veränderung des HbA1c vom Ausgangswert in Woche 24. Sekundäre Endpunkte waren die mittleren Veränderungen von Körpergewicht, Nüchternplasmaglukose und systolischem Blutdruck. Randomisierung erfolgte stratifiziert nach Antidiabetika-Hintergrundtherapie (10 mg DAPA: n = 160; PBO: n = 161). Ergebnisse: Die Ausgangswerte waren zwischen den Behandlungsarmen ausgeglichen. In Woche 24 reduzierte DAPA vs. PBO signifikant HbA1c (–0,34% [95%-KI: –0,53; –0,15]; p < 0,001), Körpergewicht (–1,43% [–2,15; –0,69]; p < 0,001), Nüchternplasmaglukose (–16,59 mg/dL [–26,73; –6,46]; p = 0,001) und systolischen Blutdruck (–3,1 mmHg [–6,3; 0,0]; p < 0,05). Nach 4, 12 und 24 Wochen war DAPA mit eGFR-Reduktionen vom Ausgangswert vs. PBO assoziiert. In Woche 27 kehrte die eGFR zum Ausgangswert zurück. Insgesamt war DAPA gut verträglich. Unerwünschte Ereignisse (UE) waren zwischen DAPA und PBO ausgeglichen (UE: 11,9 vs. 17,8%; schwerwiegende UE: 5,6 vs. 8,7%). Harnwegsinfektionen, Genitalinfektionen und Hypoglykämien waren ähnlich häufig. Schlussfolgerungen: Diese Ergebnisse bekräftigen das positive Nutzen/Risiko-Profil von DAPA bei Patienten mit T2D und CKD-Stadium 3A.
