L’itinérance chez une cohorte de jeunes adultes avec premier épisode psychotique : étude comparative à 2 ans de l’impact de l’itinérance sur l’évolution fonctionnelle et symptomatique

CONTEXTE La psychose augmente le risque d’itinérance, menant à des morbidité et mortalité accrues. Les jeunes adultes avec premier épisode psychotique (PEP) sont davantage vulnérables. Or, la littérature s’intéressant à l’itinérance chez les PEP est ténue. OBJECTIFS Comparaison de l’évolution clinique et fonctionnelle du groupe PEP avec itinérance versus celui sans itinérance, à l’admission et deux années suivant l’admission à une Clinique d’intervention précoce pour psychose (CIP). MÉTHODOLOGIE D’octobre 2005 à avril 2011, 167 PEP admis de façon consécutive (18-30 ans) sont inclus dans une étude de suivi longitudinal prospective d’une CIP du centre-ville de Montréal, Canada. Les caractéristiques sociodémographiques, symptomatiques et fonctionnelles, les traitements et l’utilisation des services, sont recueillis à l’admission puis annuellement. RÉSULTATS 26% présentent de l’itinérance, avant ou pendant le suivi. L’attrition est similaire entre les groupes. À l’admission, les itinérants présentent davantage de traumatismes dans l’enfance, d’antécédents judiciaires, psychose non-affective, symptômes négatifs, toxicomanie et personnalité du cluster B. Malgré l’intensité du suivi, une sévérité de la maladie et une adhésion à la médication similaires à l’admission, les jeunes avec vécu d’itinérance présentent une évolution clinique et fonctionnelle plus faible à 2 ans, et ce malgré davantage d’antipsychotiques injectables (vs oraux), d’ordonnances de soins et d’hospitalisations psychiatriques. CONCLUSION L’itinérance est un phénomène prévalent chez les PEP, associé à une évolution clinique et fonctionnelle moins favorable. Davantage d’études sont requises pour développer des interventions adressant les facteurs potentiellement modifiables (par exemple : toxicomanie, support social). DÉCLARATION D'INTÉRÊT Aucune.
