Effet néphroprotecteur de l’agalsidase alfa : suivi à 12 ans de patients masculins atteints de la maladie de Fabry

Introduction Notre objectif était d’évaluer l’impact du traitement par agalsidase alfa sur le déclin du DFGe chez les patients atteints de la maladie de Fabry dans le registre Fabry Outcome Survey (FOS). Méthodes Nous avons analysé des patients de plus de 16 ans au début de leur traitement par agalsidase alfa (baseline : BL), avec une durée de traitement minimale de 7 ans, au moins 3 mesures de DFGe disponibles, aucun antécédent de dialyse/transplantation et un DFGe à la BL ≥ 45 ml/min/1,73m 2 . Les patients ont été stratifiés en deux cohortes : atteinte rénale élevée (high renal involvement HRI, protéinurie de BL > 0,5 g/24 h) et atteinte rénale faible (Low Renal Involvement LRI, protéinurie de BL ≤ 0,5 g/24 h). Le DFGe a été calculé à l’aide de la formule CKD-EPI. 138 hommes ont été inclus (108 LRI, 30 HRI), âge moyen 34,0 ans à l’inclusion (intervalle : 16,7–75,9). Résultats obtenus ou attendus La moyenne ±écart type (ET) de la durée du traitement était de 12,4 ± 3,1 années (intervalle : 7,0–18,1) et de 12,3 ± 3,2 années (intervalle : 7,0-17,5) pour LRI et HRI, respectivement. À la BL, la moyenne ± ET du DFGe était de 106,9 ± 22,6 ml/min/1,73m 2 pour LRI vs 96,4 ± 25,0 ml/min/1,73m 2 pour HRI ; La protéinurie moyenne ± ET était de 0,19 ± 0,12 g/jour pour LRI vs 1,23 ± 0,92 g/jour pour HRI. Le taux annuel moyen de variation du DFGe ± l’erreur type de la moyenne (ETM) était de − 1,43 ± 0,18 ml/min/1,73m 2 pour le groupe LRI par rapport à − 2,40 ± 0,36 ml/min/1,73m 2 pour le groupe HRI. Conclusion L’utilisation de l’agalsidase alfa peut être associée à de potentiels effets néphroprotecteurs, conduisant à une stabilisation de la fonction rénale par rapport au déclin attendu chez les patients atteints de maladie de Fabry. Ces résultats peuvent également suggérer que la protéinurie de base est un facteur de risque d’insuffisance rénale.
