Proximal Flexor Digitorum Superficialis Calcification Causing Ulnar Neuralgia at the Elbow Treated with Ultrasound-Guided Lavage

Abstract
Distal flexor digitorum superficialis (FDS) calcific tendinopathy has been described in the fingers [1], even though it remains rare compared with other locations of calcific tendinopathy. However, there is no mention in the literature of proximal FDS calcific tendinopathy. Ultrasound (US) examination of a 52-year-old woman with progressive atraumatic right ulnar neuralgia revealed ulnar nerve mild focal swelling at the cubital tunnel and a 9×13×6-mm calcification within the tendinous portion of the proximal humeroulnal head of the FDS (Figure 1; Supplementary Video 1). An anteroposterior x-ray of the right elbow before the procedure was also done (Figure 2). A dynamic US study showed impingement of the ulnar nerve over the calcification with elbow flexion (Figure 1; Supplementary Video 1). ... An US-guided percutaneous lavage of the calcification was performed (Figure 3; Supplementary Video 1), immediately followed by a 40-mg methylprednisolone injection within the proximal humeroulnar head of the FDS tendon. A control x-ray four weeks later was compatible with the resorption of calcifications.
