A preterm neonate with diffuse rash at birth

Abstract
A preterm neonate was noted to have an extensive maculopapular rash at birth, which evolved into a papulopustular rash within hours. The pustules affected the face, trunk and extremities, and appeared as pinpoints on an erythematous base. The texture was sandpaper-like; palms and soles were not involved. Vital signs were within normal limits. Screening investigations for neonatal sepsis were unremarkable. On postnatal day (PND) 1, the rash became progressively confluent and erythematous (Figure 1). Blood cultures were drawn (Table 1) and empiric ampicillin and gentamicin were started. ... ... The baby was born via spontaneous vaginal delivery at 30 weeks and 2 days gestation to a healthy 33-year-old G3P1 mother. Maternal serologies were protective. Rupture of membranes occurred at delivery. The baby weighed 1.715 kg (appropriate for gestational age), and was vigorous with APGAR scores of 8 and 9, at 1 and 5 minutes, respectively. On PND 2, a pathologist contacted the neonatal intensive care unit (NICU) regarding the microscopic examination of the umbilical cord, which demonstrated acute chorioamnionitis, chorionic vasculitis and umbilical cord phlebitis. Triangular shaped microabscesses with their bases adjacent to the perimeter of the umbilical cord were present.
