Abstract
Purpose: A 54-year old woman presented with a two-day history of nausea, vomiting and abdominal pain. Her past history was significant for melanoma of the right lower limb, first diagnosed in 2006 and treated surgically. There was local recurrence in 2007 treated surgically again. In 2009 she was found to have metastases to her uterus requiring hysterectomy. Later that year, she had right mastectomy and radiation therapy for breast metastases. In 2010 brain metastases were treated with radiation therapy and subsequently surgery. At the same time, pulmonary and cutaneous metastases were noted. On this occasion her AST peaked at 230 IU/L, ALT 336 IU/L and bilirubin 178 μmol/L. CT scan showed a dilated CBD of 1.8 cm and a distal mass. ERCP was performed and a large, oblong, distal filling defect was seen on fluoroscopy (Image 1). Following sphincterotomy, the duct was explored with a balloon catheter. A red, fleshy mass was extracted and subsequently retrieved with a Roth net (Image 2). Histological analysis confirmed this to be melanoma. Her symptoms resolved and bilirubin fell to 39 μmol/L. Unfortunately, she succumbed to her illness 2 weeks later. Melanoma has previously been reported to metastasize to the CBD. This case is unique in that it is the first ever report of intraductal melanoma actually being extracted from the duct during ERCP.
