P1‐252: NEURODEGENERATION MEDIATES AND MODERATES THE EFFECT OF AMYLOID AND TAU ON LONGITUDINAL COGNITION IN MILD COGNITIVE IMPAIRMENT

Abstract
While recent literature proposes a temporal evolution of amyloid-β (A), tau (T) and neurodegeneration (N) in Alzheimer's disease (AD), the interplay of ATN pathophysiology on cognition in mild cognitive impairment (MCI) remains unclear. Given that abnormalities in CSF AD biomarkers occur early and precede their respective imaging biomarkers, CSF constitute a valuable strategy to evaluate the causative relationship between ATN pathophysiology and cognitive symptoms in the MCI stage. 319 MCI individuals with baseline and 2-year CSF and neuropsychological assessments were studied. Mediation and moderation analysis evaluated the role of neurodegeneration in the effects of amyloid-β and tau leading to memory and executive function (EF) decline at 2 years. A mediating effect of neurodegeneration is observed: higher tau is related to poorer memory function at 2 years via higher neurodegeneration (Figure). Neurodegeneration also moderates the effects of tau on memory function and amyloid-β on EF at 2 years. When neurodegeneration is low, higher tau is associated with poorer memory and greater decline in memory over 2 years, while higher amyloid-β load is associated with greater decline in EF over 2 years.
