TPS 791: Occupational health 1, Exhibition Hall, Ground floor, August 26, 2019, 3:00 PM - 4:30 PM Background: Radon is an established lung carcinogen, and findings of occupationally exposed miners have played an important role in characterizing the exposure-response relationship curve. Nonetheless, there remain uncertainties regarding risks associated with coronary heart diseases, including risks at low-levels of exposure, interactions with cigarette smoking, and changes in risk with increasing latency. To address these research questions, we performed an analysis of a cohort of Newfoundland Fluorspar miners whose mortality experience was followed up from 1950 through 2016. Methods: Analyses were performed on 2,121 miners who were employed in the mining of fluorspar between 1933 and 1978 in St. Lawrence Newfoundland. Cause of death information was determined through record linkage to the Canadian Mortality Database through December 31, 2016. Cumulative radon exposure estimates in working levels months (WLM), by year, were assigned to each cohort member, and four cycles of smoking survey (1966, 1978, 1993, & 2003) were conducted. The Cox-proportional hazard ratio (HR) models were applied to characterize the risk across levels of exposure, adjusting-for cigarette smoking, and attained age. Results: A total of 378 deaths were due to coronary heart disease. The mean occupational exposure to radon was 378 WLM over an average of 5.5 exposure years. The incidence rate of coronary heart disease amongst miners exposed at higher levels compared to the lowest levels is 4.97 vs. 3.51 per 1000 person-years. The adjusted HR between these two categories of exposure is 1.53 (95% CI: 1.07 – 2.18), with no presence of effect-modification by smoking. Conclusions: The long follow-up interval of this cohort now spans nearly 8 decades and provides insight on the risks of coronary heart disease in relation to time-varying aspects of radon exposure. Some of our findings should be interpreted cautiously due to increased exposure measurement error during the earlier stages of the follow-up period.
