ZUSAMMENFASSUNG Kriterien für die Diagnose eines LADA sind eine Manifestation nach dem 30. Lebensjahr, Nachweis von Insel-Autoantikörpern und die Möglichkeit, mindestens 6 Monate ohne Insulin zu therapieren. Das C-Peptid ist meist niedrig normal bzw. erniedrigt. Die Häufigkeit des LADA bei Patienten mit dem klinischen Phänotyp eines Typ-2-Diabetes liegt bei ca. 10 %. Bei GAD-Antikörper-positivem Befund müssen binnen 6 Jahren 40 % der Patienten im Alter von über 45 Jahren und 70 % der Patienten unter 45 Jahre mit Insulin behandelt werden. Die verfügbare Datenlage erlaubt keine eindeutigen Schlüsse bezüglich der optimalen initialen medikamentösen Therapie des LADA. Sulfonylharnstoffe sind ungünstiger als Insulin. Die Behandlung mit DPP-4-Hemmern scheint bei LADA ähnlich gut wirksam zu sein wie bei Typ-2-Diabetes. Erste Daten zu Dulaglutid sind positiv, sodass auch GLP-1-Analoga eine Therapiealternative sind.
