악성 간문부 담도 협착의 스텐트 삽입에 있어 Soehendra Stent Retriever의 유용성

Abstract
목적: 악성 간문부 담도 협착의 고식적 치료로 내시경적 담도 배액술이 널리 사용되고 있다. 고도의 담도 협착의 경우는 확장도관이나 풍선도관 등의 일반적인 방법으로는 담도 확장이 어려워 스텐트 삽입에 실패하는 경우가 있다. 저자들은 일반적인 내시경적 방법으로 스텐트 삽입이 불가능 했던 악성 간문부 담도 협착 환자에서 Soehendra Stent Retriever를 사용하여 담도 확장 및 스텐트 삽입을 시행하여 그 유용성을 평가하였다. 대상 및 방법: 2006년 7월부터 2008년 6월까지 67명의 악성간문부 담도 협착 환자 중 확장도관이나 풍선도관으로 담도 확장이 어려운 12명(간문부 담도암=10명, 담낭암=2명)을 대상으로 하였다(남:여=7:5, 평균 연령=69.1세). 양측 담도 배액을 위하여 in stent 방법을 이용하였으며, ERCP 카테터나 스텐트가 유도 철선을 따라 협착부를 통과하지 못한 12명의 환자에서 Soehendra Stent Retriever를 사용하여 시계방향으로 회전시킴과 동시에 전진시켜 원하는 간내 담도 협착 부위를 확장시킨 다음 금속 스텐트를 삽입하였다. 결과: Soehendra Stent Retriever를 사용한 12명의 환자 중 10명에서 스텐트 삽입(기술적 성공률, 83.3%)에 성공하였고, 이들 모두에서 황달의 호전을 보였다. 시술 후 발열과 췌장염이 있었지만 보존 치료로 호전되었으며 심각한 합병증은 관찰되지 않았다. 결론: 고도의 악성 간문부 담도 협착 환자에서 양측성 스텐트 삽입시 Soehendra Stent Retriever를 사용하여 협착 부위를 확장할 경우 스텐트 삽입의 성공률을 높일 수 있다.
