AbstrakDiabetes melitus (DM) adalah kelainan metabolik yang ditandai dengan kenaikan kadar guladarah akibat kerusakan sekresi insulin. Komplikasi DM yang paling banyak adalah  neuropatidiabetik perifer (NDP) dengan prevalensi 50-60%. Dislipidemia aterogenik merupakanfaktor resiko metabolik sekaligus vaskuler yang berperan pada patogenesis NDP. Kombinasipenilaian profil lipid  dengan rasio lipid TG/HDL (indeks aterogenik) dapat mengetahuiadanya aterosklerosis yang tersembunyi dan berhubungan dengan derajat keparahan NDP.Peneliti ingin menganalisis hubungan indeks aterogenik plasma terhadap derajat keparahanneuropati diabetik perifer pada penderita DM tipe 2. Penelitian ini menggunakan desainanalitik observasional potong lintang  dari bulan November 2018 – Januari 2019 terhadap 51penderita DM tipe 2 di bangsal Neurologi dan Penyakit Dalam serta poliklinik PenyakitDalam RSUD Dr. Moewardi Surakarta. Pemeriksaan indeks aterogenik plasma berdasarkanperhitungan lipid LogTG/HDL dan derajat keparahan NDP dengan modifed Toronto ClinicalNeuropathy Scoring (mTCNS). Data dianalisis  dengan menggunakan program SPSS 22.0.Hasil penelitian melibatkan 28 pasien laki-laki dan 23 perempuan dengan indeks aterogenik(IA) rendah 15 orang, IA sedang 9 orang dan IA tinggi 27 orang, dengan rerata 0.158. Daripengisian skor mTCNS didapatkan skor mTCNS rendah 17 orang (33.33%), skor sedang 19orang (37,2%)  dan mTCNS tinggi 15 orang (29.41%), dengan rerata 14,11 ± 8,022.  Darihasil uji regresi linier IA dengan skor mTCNS didapatkan nilai p=0,000, dengan nilaiAdjusted R2 sebesar 0,464. Dari penelitian dapat disimpulkan terdapat hubungan antaraindeks aterogenik plasma terhadap derajat keparahan NDP pada pasien DM tipe 2.Hubungan tersebut  merupakan hubungan positif (searah) yang berarti semakin tinggi indeksAterogenik maka derajat keparahan NDP akan cenderung semakin tinggi.  Kata Kunci : Derajat keparahan Neuropati Diabetik Perifer, Indeks aterogenik plasma
