Abstract
患者男，58岁。间断腹胀3年，曾伴腹泻、腹痛、恶心、呕吐，无发热、便血。外院上消化道造影示胃十二指肠多发息肉样病变。为进一步诊治来我院。患者诉3年前曾突发全身肤色加深，头发、眉毛严重脱落，双侧指（趾）甲萎缩、脱落。因疑似“砷中毒”，查血砷、尿砷含量均正常，未特殊治疗，其后上述症状有不同程度好转。既往有2型糖尿病病史；无吸烟及嗜酒史；家族中未见有相似病史者。入院体检：生命体征平稳；全身肤色较深（图1），毛发分布基本正常，双侧指（趾）甲畸形（图2）；心、肺、腹部体检无阳性体征；双下肢无水肿。辅助检查：血常规示Hb114g/L；血生化示白蛋白（Alb）28.1g/L，血清铁9.4μmol/L；反复查粪常规，均示潜血阳性，未见红细胞、白细胞及寄生虫；C反应蛋白（CRP）及ESR正常；癌胚抗原（CEA）、甲胎蛋白（AFP）、CA199、CA125均正常。复查血砷、尿砷含量无异常。胃镜、小肠镜、结肠镜检查示全消化道多发息肉（图3～5），镜下多处取活检，活检病理示：增生性息肉及管状腺瘤Ⅰ级（图6～8）。诊断：Cronkhite-Canada综合征（Cronkhite-Canadasyndrome，CCS）。
