Abstract
A 3-year-old boy presented to the emergency department with a 5-day history of worsening respiratory distress and fever. His medical history was notable for Pierre-Robin sequence, recurrent aspiration pneumonia, prematurity (33 weeks), G-tube dependency, chromosome 16 deletion, sensorineural hearing loss, neurodevelopmental disorder, and chronic lung disease requiring home oxygen. There was no recent travel history or known sick contacts. There was no reported history of choking, vomiting, or witnessed ingestions. Of note, he had been repeatedly admitted to the paediatric intensive care unit on nine different occasions over the preceding year for similar presentations. Physical exam revealed tachypnea, subcostal indrawing, congested cough, and inspiratory crackles. Initial chest x-ray revealed perihilar inflammation that had been noted previously. He was first treated for presumed aspiration pneumonia with a course of Amoxicillin-Clavulinic acid, followed by 5 days of corticosteroids and bronchodilators for asthma exacerbation. Serial chest x-rays documented bilateral lower lobe atelectasis, despite antibiotics. His respiratory support gradually escalated from high-flow nasal cannula to continuous positive airway pressure over the course of 5 days. A subsequent investigation revealed the diagnosis.
