Yaşlı hastalarda katarakt cerrahisinin kognisyona olan olumlu etkilerinin değerlendirilmesi

Abstract
Bu çalışmada \nkataraktın yaşlı hastalarda kognisyona olan etkilerinin değerlendirilmesi \namaçlanmıştır. Çalışmaya katarakt tanısı ile operasyon planlanan görme keskinliği \nkötü 60-80 yaş aralığında 30 hasta alınmıştır. Katılımcıların ameliyat öncesi \nkognitif testleri yapılmış, operasyondan 3 ay sonra aynı testler yeniden tekrar \nedilmiştir. Tüm hastalar lokal anestezi ile birer hafta arayla binokuler katarakt \ncerrahisi geçirmişlerdir. Hastaların operasyondan gördükleri fayda görme keskinliği \nile karşılaştırılmıştır. Demans taraması Montreal Bilişsel Değerlendirme Ölçeği \n(MOCA) testi ile yapılmıştır. Katılımcıların %46 sında demans , % 36 sında hafif \nkognitif yetmezlik saptanmıştır. Demansı olan katılımcıların en çok bellek, lisan, \ngörsel- uzaysal bellekte; hafif kognitif yetmezlikli hastaların dikkat, yürütücü işlev \ntestlerinde kötüleşme izlenmiştir. Katarakt cerrahisi sonrası görme keskinliği ile \nkorele şekilde kognitif yetmezlikli olguların MOCA skorlarında en az 2 puan artış \nizlenmiştir. Ancak görme keskinliğinde artış görsel bellek, konstruksiyonel praksi, \nsaat çizme, şekil kopyalama testlerindeki skorlarla korele bulunmamıştır. Hastaların \ngünlük yaşam aktivitesi, geriyatrik depresyon ölçeği entrümental günlük yaşam \naktivite skorları post operatif dönemde görme keskinliği ile korele şekilde \ndüzelmiştir. Sonuç olarak kognitif yetmezlik ve katarakt yaşlı populasyonda sık \nbirliktelik gösteren hastalıklardır. Görme yetisinin yeniden kazandırılması \nkognisyonu olumlu şekilde etkileyebilir. Demans polikliniklerinde hastaların duysal \nkayıpları ile birlikte değerlendirilmesi hastalığın takibi açısından önemlidir.
