Introduction : Les lignes esthétiques sont des paramètres céphalométriques de référence proposées dans les années 60 et connues de tous les orthodontistes. Les plus utilisées sont celles de Ricketts, Steiner et Burstone. Elles formalisent de façon simple l’évaluation du profil labial cutané dans le sens sagittal et déterminent le diagnostic de prochélie, normochélie ou rétrochélie. Cependant, nous pouvons nous interroger sur la pertinence de ces références. L’objectif de cette étude observationnelle transversale est d’évaluer leur légitimité afin de guider le plan de traitement orthodontique et l’indication de repositionnement dentaire selon l’âge du patient. Matériel et méthode : L’échantillon est composé de 96 patients sans limite d’âge dont les téléradiographies sont sélectionnées par stratification aléatoire. Les trois critères de jugement principaux sont les lignes E de Ricketts, S de Steiner et B de Burstone. Les mesures sont effectuées par un seul opérateur via le logiciel IMAGEJ. Résultat : D’après l’analyse de Spearman, plus le sujet est âgé, plus la rétrochélie augmente. Ceci avec une forte fiabilité pour les critères de Ricketts (rho>0,6) et Steiner (rho>0,6). De plus, l’âge pivot à 25 ans est vérifié par le test de Wilcoxon-Mann-Whitney (p < 0,05). Conclusion : L’utilisation des lignes esthétiques E, S et B comme argument d’un repositionnement dentaire chez l’adolescent et l’adulte jeune est injustifiée, de par l’absence de prise en compte du recul labial physiologique avec l’âge.
