Abstract
A healthy 15-month-old male presents to a community hospital with a 4-day history of lethargy, vomiting, and decreased appetite. There is no report of fever, infectious symptoms, or trauma. He appears tired and dehydrated. Blood pressure is 107/67, heart rate is 94 bpm, and temperature is 36.8°C. Examination reveals normal cardiac, respiratory, and abdominal examinations, no focal neurologic deficits and no skin rashes or bruises. He is given oral rehydration and paediatrics is consulted for admission. When the paediatrician arrives, the patient wakes and cannot open his right eye. His right pupil is fixed and dilated and his blood pressure and heart rate are now 121/60 and 80 bpm, respectively. Increased intracranial pressure is suspected so a 5 mL/kg bolus of 3% saline is given and a CT head is done, which shows a large right subdural hemorrhage (SDH) with mass effect and parenchymal effacement (Figure 1). The patient is transferred to a tertiary centre for surgical intervention and consultation with the hospital child maltreatment team. Preliminary bloodwork is drawn and results are shown in Table 1.
