Le concept d'HAD (hospitalisation a domicile) est apparu en France il y a presque soixante ans. Initialement, l'HAD etait destinee aux patients cancereux ou en fin de vie. La circulaire de 2004 a elargi son champ d'action a l'obstetrique. Des experiences d'HAD obstetricale ont ete menees avec succes a l'etranger (Canada, Pays-Bas) mais aussi en France (surtout pour le post-parturn). Afin d'evaluer la viabilite d'une HAD obstetricale sur la region messine, une enquete a ete realisee au sein d'une clinique, durant un an, sur la prise en charge des menaces d'accouchement premature (MAP) , qui est le premier motif d'hospitalisation en ante-partum. Ce travail a pour buts de determiner la proportion des differentes categories de MAP en fonction de leur degre de severite et d'evaluer quelle part de ces patientes auraient pu beneficier de l'HAD (si elle avait existee) en premiere intention ou apres une courte hospitalisation, en tenant compte de criteres medicaux mais aussi sociaux et geographiques. Sur les 111 dossiers retenus, 60 % des femmes pouvaient pretendre a une HAD quasi immediate et 25 % apres une hospitalisation traditionnelle de 5 jour. Cette etude montre l'interet de l'HAD comme structure de prise en charge intermediaire entre soins ambulatoires et hospitalisation traditionnelle. Par ailleurs, ce mode de prise en charge est en phase avec les principaux objectifs du plan perinatalite 2005-2007 et les aspirations des patientes et de leur famille.
