An Inside Perspective of the Opioid Overdose Crisis in Vancouver: A Secondary Qualitative Study

Abstract
ABSTRACT Objectives: To explore the perceptions of the opioid crisis among individuals living in poverty and using illicit substances in the Downtown Eastside (DTES) of Vancouver. Design: A qualitative design using secondary analysis of semi-structured interviews. Setting: This study was set in Vancouver's DTES in 2017. This neighbourhood is often considered “the poorest postal code in Canada”, and has a history of substance use and marginalization. There were a record number of opioid overdoses in the DTES during this time. Participants were recruited from peer harm-reduction agencies and were currently using opioids or had previous experience using opioids. Methods: Semi-structured interviews were audio-recorded and transcribed. Secondary thematic analysis was performed in an iterative manner to identify major themes. Findings: Transcripts of 33 interviews were analyzed. Four major themes were identified: (1) the overwhelming trauma of living within the opioid crisis with respect to both the suddenness and number of deaths, (2) a community response including activism, harm reduction, and a responsibility to save others from overdose, (3) the marginalization and stigma that the population of individuals using substances continues to face, and (4) the importance of recognizing human rights as drug user rights. Conclusions: DTES residents have been heavily impacted by the overwhelming trauma of the opioid crisis, but participants also spoke of community responsiveness and responsibility. Based on these findings, peer harm reduction training programs should account for the potential emotional impact in the training of new peers, and trauma-informed care should acknowledge that many members of this community are grieving. Conception: Une conception qualitative utilisant une analyse secondaire d’entretiens semi-structurés. Contexte: Cette étude a été réalisée dans le DTES de Vancouver en 2017. Ce quartier est souvent considéré comme «le code postal le plus pauvre du Canada» et a des antécédents de consommation de substances et de marginalisation. Il y a eu un nombre record de surdoses d’opioïdes dans le DTES pendant cette période. Les participants ont été recrutés dans des organismes homologues de réduction des méfaits et utilisaient actuellement des opioïdes ou avaient déjà utilisé des opioïdes. Méthodes: Les entretiens semi-structurés ont été enregistrés et transcrits. Une analyse thématique secondaire a été réalisée de manière itérative pour identifier les grands thèmes. Résultats: Les transcriptions de 33 entrevues ont été analysées. Quatre thèmes majeurs ont été identifiés: (1) le traumatisme accablant de vivre dans la crise des opioïdes en ce qui concerne à la fois la soudaineté et le nombre de décès, (2) une réponse communautaire comprenant l’activisme, la réduction méfaits et la responsabilité de sauver les autres d’une surdose, (3) la marginalisation et la stigmatisation auxquelles la population des consommateurs de drogues continue de faire face, et (4) l’importance de reconnaître les droits de l’homme en tant que droits des consommateurs de drogues. Conclusions: les résidents du DTES ont été fortement touchés par le traumatisme écrasant de la crise des opioïdes, mais les participants ont également parlé de la réactivité et de la responsabilité de la communauté. Sur la base de ces résultats, les programmes de formation sur la réduction des méfaits par les pairs devraient tenir compte de l’impact émotionnel potentiel de la formation des nouveaux camarades, et les soins tenant compte des traumatismes devraient reconnaître que de nombreux membres de cette communauté sont en deuil.
