The Second Short Form of the McGill Pain Questionnaire as the useful additional tool for differential diagnostics of migraine and cervicogenic headache

Abstract
ЦЕЛЬ ИССЛЕДОВАНИЯ Изучение эффективности Второй сокращенной формы Макгилловского болевого опросника в качестве инструмента оценки и сравнения количественных характеристик боли у пациентов с неспецифической болью в шее, сочетающейся с нарушениями биомеханики шейного отдела позвоночника и сопровождающейся цервикогенной головной болью (ЦГБ) или мигренью. МАТЕРИАЛ И МЕТОДЫ Сорок девять пациентов в возрасте от 19 до 60 лет (средний возраст 42,8±10,4 года) составили сопоставимые по возрасту и половому признаку группы больных мигренью (n=19) и ЦГБ (n=31). Интенсивность боли в шее и головной боли оценивали по 11-балльной числовой рейтинговой шкале (ЧРШ); интенсивность дескрипторов боли, связанных с ней неприятных ощущений и аффективных реакций — по SF-MPQ-2. Лечение проводили в рамках мультидисциплинарного, мультимодального подхода, комбинируя лекарственное и нелекарственное лечение, включающее различные методики мануальной терапии. РЕЗУЛЬТАТЫ Интенсивность головной боли в группе больных мигренью была достоверно выше (p<0,05), чем в группе ЦГБ (7,8±1,7 против 6,0±1,5 соответственно); различий интенсивности боли в шее не выявлено (6,2±1,8 против 5,3±1,7; p=0,08). Больные мигренью выбирали достоверно большее количество дескрипторов (p<0,001), чем больные ЦГБ (14 против 4 соответственно), и более высокий ранг их интенсивности, что можно объяснить различиями в патогенезе мигрени и ЦГБ. В результате лечения количество дескрипторов и ранг их интенсивности уменьшились в обеих группах по всем 4 шкалам SF=MPQ-2 (p<0,001), что можно объяснить устранением цервикогенной составляющей боли. Удовлетворение результатами лечения выразили 83% больных мигренью и 97% пациентов с ЦГБ, отметив по шкале Общего воспринимаемого эффекта дескрипторы «лучше» и «гораздо лучше». ЗАКЛЮЧЕНИЕ Опросник SF-MPQ-2, содержащий необходимые дескрипторы и шкалу, позволяющую оценить их интенсивность, может быть успешно применен для дифференциальной диагностики мигрени и ЦГБ.
