Antecedentes: La CRTO (Cirugia Robotica Trans Oral) es una de las ultimas alternativas quirurgicas en el tratamiento de los car-cinomas de celulas escamosas de orofaringe (CCE-OF) y cada vez mas aceptado por los cirujanos. Los robots quirurgicos fueron disenados para permitir la telecirugia y eliminar factores humanos como el temblor, entre otros moitvos. El estudio tuvo como objetivo revisar la literatura disponible para evaluar la seguridad y eficacia de la CRTO en comparacion con la cirugia abierta.Material y metodos: Realizamos una revision sistematica para evaluar la seguridad y eficacia de la CRTO en comparacion con la cirugia abierta. Analizamos ambas cirugias segun 16 parametros divididos en 3 grupos: complicaciones intraope-ratorias, complicaciones postoperatorias y resultados funcionales y oncologicos. Se realizo una busqueda electronica de estudios observacionales utilizando las siguientes bases de datos: MEDLINE, EMBASE, Registro Cochrane Central de Ensayos Controlados, Registro de Ensayos del Grupo Cochrane de Salud Oral y Scielo. El analisis de los datos se realizo de acuerdo con los Preferred Reporting Items for Systematic Reviews and Metanalysis (PRISMA) y la calidad de los estudios se evaluo mediante la escala de Newcastle-Ottawa. No se impusieron restricciones de idioma.Resultados: En los 4 estudios identificados (puntuacion media en la escala de Newcastle-Ottawa de 6,5), se revisaron 371 pacientes (186 pacientes fueron tratados con CRTO y 185 con cirugia convencional). En general, la CRTO vs la cirugia abierta, parece tener mejores resultados funcionales (menor tiempo de hospitalizacion y decanulacion) y menor complica-ciones intra y postoperatorias. No hay diferencia significativa al evaluar los resultados oncologicos (margenes positivos, tasa de supervivencia) al comparar ambas tecnicas. Conclusiones: La CRTO tiene un mejor resultado funcional en general, y menor complicaciones intraoperatorias y postoperatorias. No hay diferencias en los margenes positivos ni tampoco en la tasa de supervivencia en comparacion con la terapia convencional.
