Identification et validation des compétences cliniques minimales chez les infirmières débutantes en soutien à domicile

Abstract
RÉSUMÉ: La présente étude avait pour but d’identifier et de valider les compétences cliniques minimales que devraient idéalement posséder les infirmières débutantes en soutien à domicile (SAD) afin d’assumer leur rôle professionnel au sein du réseau de la santé et des services sociaux du Québec. Ce processus de validation a été conduit en trois étapes. Il s’agit de la définition du construit de compétence clinique de l’infirmière débutante en SAD, de la génération d’un échantillon d’énoncés de compétences et de la validation a priori des énoncés de compétences auprès de gestionnaires et d’infirmières expertes en SAD. La définition du construit de compétence clinique de l’infirmière débutante en SAD a permis de retenir 11 dimensions permettant de classifier l’ensemble des compétences cliniques de l’infirmière en SAD. Il s’agit de l’évaluation, la surveillance et la prise de décision clinique, la planification et la coordination des soins, le maintien de la santé et la palliation, l’enseignement, la communication, la relation thérapeutique, l’accès et l’équité des soins, la promotion de la santé, la prévention de la maladie, la qualité des soins et les responsabilités professionnelles.La génération d’un échantillon d’énoncés de compétences a permis de recenser (approche déductive) plus de 94 compétences pouvant potentiellement représenter les compétences cliniques minimales que devraient posséder les infirmières débutantes en SAD. Chacune de ces compétences a été formulées sous forme d’énoncés et soumis à une pré-validation de leur clarté auprès de trois infirmières expertes en SAD agissants à titre de juges avant les soumettre officiellement aux experts. Par la suite, la validation a priori a été réalisée auprès d’un échantillon de 12 gestionnaires en SAD et de 15 infirmières expertes en SAD qui devaient maintenant se prononcer sur la clarté, la pertinence et l’importance de chacun des énoncés de compétences. Ce processus de validation comportait également une approche inductive permettant aux experts de soumettre d’autres idées de compétences qui leur apparaissaient pertinentes et importantes. Au terme de ce processus qui a nécessité deux consultations des experts, 105 énoncés de compétences ont été validés méthodologiquement. L’identification ainsi que la validation de ces énoncés de compétences permettront, entre autres, aux gestionnaires d’offrir des programmes de mentorat ainsi que des formations spécialisées en cours d’emploi favorisant le développement des compétences. This process required three steps. These steps refer to the definition of the competencies construct of advanced beginner home care nurses, the generation of a sample of competence statements and the validation a priori of the competency’s statements by the managers and expert nurses in home healthcare. Each of these skills were formulated in an interrogative form and subjected to a pre-validation by three experienced nurses in home care services with the purpose of establishing the clarity of statements before officially submitting to the experts. Thereafter, the content validation was carried out by a sample of 12 home health managers and 15 home health nurses who were to come to consensus about the clarity, the relevance and the importance of each competence statements. This process of validation also comprised an inductive approach making it possible for the experts to propose other ideas of skills that they found relevant and important to them. At the end of this process which required two consultations with experts, 105 competence statements were methodologically validated. The identification and the validation of these competencies’ statements will allow home health managers to offer on job training programs that promote the development of clinical skills.
