Abstract
An eight-year-old girl presented with a three-week history of vaginal bleeding associated with lower abdominal cramps. She had a three-month history of breast and pubic hair development. She had no axillary hair or odour. She was previously well and on no medications. She had no history of headaches, galactorrhea, constipation, weight gain or dry skin. On examination, she was at the 97th percentile for height and the 90th percentile for weight, at 138 cm and 33 kg, respectively. Her height had increased 9 cm in the previous year. She had pubic hair at Tanner stage II and breasts at Tanner stage III. Her abdomen was soft and non-tender, with no palpable masses. The rest of the examination was unremarkable. A bone age test revealed a skeletal age of 10 years. An ultrasound of the abdomen showed a cystic mass with a solid component, arising from the right ovary, measuring 8 cm × 5 cm × 7 cm. The uterus was postpubertal in size and the contralateral ovary appeared normal. Laboratory results showed an estradiol level of 811 pmol/L (normal prepubertal level is less than 30 pmol/L). Serum luteinizing hormone and follicle-stimulating hormone were suppressed (less than 0.3 IU/L). Testosterone, dehydroepiandrosterone sulfate, 17-hydroxyprogesterone, prolactin and thyroid-stimulating hormone levels were all normal. Serum inhibin, beta-human chorionic gonadotropin (hCG) and alpha-fetoprotein levels were normal. A diagnostic test revealed the diagnosis.
