Abstract
従来は「軟骨の変性・摩耗による非炎症性疾患」と考えられていた変形性関節症だが，新たな知見の集積によって病態解明が進み，現在では軟骨だけでなく骨，靱帯，滑膜，筋肉，神経が発症から進行まで複雑に関与する“wholejoint disease”として捉えられ，その発症および進行に炎症が関与していることが明らかになっている1)。 変形性股関節症の発症のリスクファクターとして肥満，身体活動性，股関節形態，遺伝的要因，骨密度が，進行のリスクファクターとして年齢，肥満，股関節形態があり，発症と進行を抑えるためには体重管理と外傷の予防が重要である。近年の海外のガイドラインでは，症状緩和と機能改善に対する治療として，運動や理学療法に加えて患者教育が重要視されており，薬物療法では経口非ステロイド性抗炎症薬(non-steroidal anti-inflammatory drugs：NSAIDs)とステロイド関節内注射が推奨されている。手術療法では，人工股関節全置換術(total hiparthroplasty：THA)におけるロボット支援手術が普及し始めている。 本稿では，変形性股関節症の病態，リスクファクター，治療に関する最新知見について紹介する。
