De nombreux medicaments sont elimines par le rein et leur elimination peut etre retardee en cas d'insuffisance renale. L'accumulation des medicaments et de leurs metabolites ne survient que lorsque la fonction renale est profondement alteree, c'est-a-dire lorsque la clairance de la creatinine devient inferieure a 30-40 ml/min voire 50 ml/min. En conservant les modes de prescription utilises chez des sujets avec une fonction renale normale, on risque une accumulation dangereuse et des concentrations tissulaires anormalement elevees. L'etude quenous avons realise au Quebec montre que l'adaptation posologique des medicaments en fonction de la clairance de la creatinine du patient contribue : d'une part a l'optimisation therapeutique etd'autre part a la prevention de la iatrogenese medicamenteuse. De plus, cette etude confirme que la maladie renale est une maladie frequente qui est depistee parfois trop tardivement souvent lors d'une hospitalisation. Le pharmacien d'officine doit etre un maillon pour collaborer avec lescliniciens et les biologistes pour une prise en charge globale a condition que celui-ci ait acces a certaines informations notamment a travers un outil en devenir qu'est le Dossier Medical Personnel (DMP).
