目 的:肩関節前方脱臼に合併する関節窩骨欠損は25%以上の大きさが危険域であることが知られている.近年,13.5~20%の骨欠損(準危険域)は再脱臼しないが生活の質(QOL)を低下させるという臨床報告が出された.本研究の目的は,Hill—Sachs 損傷を評価する際にこの準危険域が存在するかどうか調査することである. 対象および方法:反復性肩関節前方脱臼に対して鏡視下Bankart 修復術を受けた25%以下の関節窩骨欠損を合併する50 例(平均年齢27 歳)を対象とした.術後平均28 ヵ月で調査を行った.全例とも軌跡内損傷である.Western Ontario Shoulder InstabilityIndex(WOSI)スコアとRowe スコアで臨床評価した.三次元CT 画像上でHill—Sachsinterval を計測し,関節窩軌跡幅で除したものをHill—Sachs 占拠率と定義した.関節窩軌跡はHill—Sachs 占拠率の大きさによって四つのzone に分けた.すなわちzone 1 は25%未満,zone 2 は25~50%,zone 3 は50~75%,zone 4 は75%以上である. 結 果:再脱臼率は6%であった.WOSI スコアもRowe スコアも術後有意に改善した.zone 4 にあるHill—Sachs 損傷(辺縁軌跡損傷10 例)のWOSI スコアはその他の損傷(中央軌跡損傷)に比べ有意に低かった.この10 例のうち5 例は術前のWOSI スコアと同じ程度に低い値であった. 結 論:軌跡内損傷の患者は,再脱臼しないがWOSI スコアが有意に低い辺縁軌跡損傷(Hill—Sachs 占拠率が75%以上)とそれ以外の中央軌跡損傷(75%未満)の二つに分けられた.
