Introduccion: se estan llevando a cabo reformas en los sistemas de salud publica de varios paises desarrollados, pero no siempre logran dar los resultados deseados. En este articulo, presentamos la historia de la reforma del sistema de salud en la provincia canadiense de New Brunswick, con el objetivo de analizar su dificil conduccion por parte del Estado, a la luz de las dinamicas que se han establecido entre los actores involucrados. Metodologia: se selecciono un metodo de investigacion cualitativa. La recopilacion de datos incluye principalmente entrevistas semiestructuradas (n = 39) con representantes del gobierno, como los ministros de Salud, y otras partes interesadas en la reforma, como gerentes, ciudadanos o profesionales. Resultados: las estrategias adoptadas por los actores clave de la reforma, que fueron impulsadas por temas especificos como la atencion y los servicios de salud en frances, influyeron en la evolucion del cambio. Para mantener el rumbo, el Estado se ha visto obligado a adaptarse a sus dinamicas relacionales. Conclusion: la reforma de un sistema de salud publica se desarrolla en un largo horizonte temporal y su direccion por parte del Estado es muy dificil, ya que requiere la movilizacion de diferentes tipos de instrumentos que poner a su disposicion. Para influir en el comportamiento de los agentes interesados, el Estado debe definir un objetivo cuyas orientaciones generales se acuerden, debe lograr crear vinculos de confianza y gestionar las resistencias y, por ultimo, debe utilizar datos normalizados para garantizar un marco normativo y evaluar la evolucion del proyecto de reforma.
