Abstract
บทคดยอ บทนำ : ยา Tacrolimus เปนยากดภมคมกนในกลม calcineurin inhibitors มประสทธภาพดในการปองกนการเกดการสลดกราฟท(graft rejection) ในผปวยปลกถายไตและตบ แตการใชยามขอจำกดเนองจากอาการไมพงประสงคทเกดขน และมรายงานวาเภสชจลนศาสตรของยา tacrolimus มความแตกตางระหวางบคคลสง งานวจยนจดทำขนเพอประเมนระดบยา tacrolimus กอนใหยาครงถดไปในผปวยปลกถายไตมากกวา 3 เดอนโดยวดรอยละของระดบยาท trough concentration ทอยในชวงการรกษา (5-15 ng/ml) วดรอยละการเพมขนของซรมครเอทนนและหาความสมพนธระหวางระดบยากบการเพมขนของซรมครเอทนน วธการศกษา : เปนการศกษาแบบยอนหลง รวบรวมขอมลผปวยปลกถายไตมากกวา 3 เดอนทเขารบการรกษาในโรงพยาบาลศรนครนทร และไดรบยา tacrolimus ทมการตรวจวดระดบยา ระหวางวนท 1 มถนายน พ.ศ. 2554 ถง 31 พฤษภาคม พ.ศ. 2555 จากผปวยปลกถายไตทงหมด 186 คน เขาเกณฑการศกษาและนำมาวเคราะหขอมลจำนวน 152 คน มการตรวจวดระดบยา tacrolimus ในเลอดทงหมด 954 ครง หาความสมพนธระหวางระดบยากบการเพมขนของซรมครเอทนน โดยใช odd ratio ผลการศกษา: พบระดบยาทวดไดอยในระดบตำกวาชวงการรกษา จำนวน 692 ครง (72.54%) ซงมระดบซรมครเอทนนเพมขนรอยละ 25-50 และมากกวารอยละ 50 จากคาพนฐานของผปวยอย 17.2% และ 14.74% ตามลำดบ พบระดบยาในชวงการรกษา จำนวน 261 ครง ( 27.35%)มระดบซรมครเอทนน เพมขนรอยละ 25-50 และมากกวารอยละ 50 อย 18.01% และ 11.11% ตามลำดบ และระดบยาสงกวาชวงการรกษา จำนวน 1 ครง (0.10%) เมอหาความสมพนธพบวาระดบยาทตำกวาชวงการรกษามแนวโนมตอการเพมขนของซรมครเอทนนไมแตกตางจากกลมของระดบยาทอยในชวงการรกษา (OR=1.132; 95%CI 0.829-1.546) และไมมนยสำคญทางสถต(p>0.05) มผปวยจำนวน 6 คนทไดรบการวนจฉยวาเกดการสลดกราฟท ดวยวธ biopsy มระดบยาอยตำกวาชวงการรกษา 29 ครง และอยในชวงการรกษา 3 ครง สรปผลการศกษา : ระดบยาสวนใหญอยในระดบตำกวาชวงการรกษา แตไมพบความสมพนธระหวางระดบยา tacrolimus ทลดลงกบการเพมขนของซรมครเอทนน จงควรทำการศกษาตอไปถงรายละเอยดของปจจยตางๆทอาจสงผลกระทบตอการทำงานของไตโดยทำการศกษาแบบไปขางหนา(prospective) เพอศกษาถงชวงการรกษาทเหมาะสมในแตละชวงเวลาหลงจากไดรบการปลกถายไตในกลมผปวยเชอชาตไทย คำสำคญ : tacrolimus; ผปวยปลกถายไต; ชวงการรกษา
